Confined Space IncidentWhen required to work in a confined space I will:
- Only conduct confined space tasks if Trained, Competent and Authorised
- Identify, Isolate, Lock and Verify all energy sources prior to entry
- Verify the rescue plan is effective, with correct equipment and trained standby person in place
- Confirm my atmospheric gas monitor is bump tested and calibrated prior to use
- Conduct atmospheric monitoring before and during my task
- Ensure mechanical or natural ventilation is adequate to purge or ventilate any contaminant prior or during task

Engulfment Incident
When there is the potential for engulfment during the task I will:
(Stockpile operations)- Check stockpile condition and confirm valve status before starting work
- Never travel over an operating coal valve
- Always maintain a full blade of coal between dozer and coal valve
- Only operate where bench height is below cab height
- Conduct pre start inspections prior to operating mobile equipment on stockpile
(Trenching)- Ensure trenches deeper than 1.5m are battered, benched or shored
- Establish suitable barriers and barricading to clearly identify and control trenching work area
- Ensure a minimum clearance zone of 1 m maintained from the trench edge.
(CHPP) -
Explosives Detonation
When working with or around explosives I will:
- Ensure no personnel are within the exclusion zone during blasting and lightning events
- Ensure blast demarcation and bunding is in place prior to the commencement of loadings
- Ensure that a traffic management plan is in place prior to loading and adhered to throughouts
- Only access, transport or handle explosives if Trained and Appointed
- Understand and follow the drill and loading plan where hot or reactive ground has been identifieds
- Immediately notify the shotfirer in charge of a suspected misfires
- Always have a shotfirer supervise any recovery or excavation involving misfires.s
- Ensure separation of explosives while being transported and only assemble initiation explosives immediately prior to placing down the holes

Exposure to Airborne Contaminants
When there is the potential for Dust or other Airborne Contaminants in my work area I will:
- Ensure I wear/fit the correct respirator type for the specific activity (consider fit-tested RPE and seal with regards to facial hair)
- Ensure I implement dust/fume suppression/mitigation controls and ensure they are in place and effective in managing further exposure to myself and others in the work area
- Ensure I verify ventilation controls are in place and effective in managing further exposure to myself and others in the work area (consider ventilation system, type and direction)

Fall from HeightWhen working at heights I will:
- Install hard barriers to protect open edges and prevent falls
- Only use equipment that’s fit for purpose, inspected, tested and tagged
- Assess ground and environmental conditions for suitability and stability
- Verify the rescue plan is effective, with correct equipment and trained standby person in place
- Always maintain three points of contact when climbing ladders, steps, or entering/exiting plant and equipment to prevent slips, trips, and falls.

Fall of GroundWhen working around dumps, highwalls, low walls and stockpiles I will:
- Inspect my work area for geotechnical hazards and report them
- Always complete a Geotechnical Operations Checklist prior to entering a Primary Standoff Zone
- Ensure primary standoff zones are demarcated to standard (minimum 10m - toe, 5m-crest)
- Ensure that windrows are constructed and maintained to the correct standard
- Dig and Dump to design

Fire, Explosion & Hazardous Substances:When working with or around hazardous substances, or working on, in or around equipment and infrastructure with the potential for fire and/or explosion I will:
- Conduct work area inspection to identify ignition sources are adequately controlled
- Conduct equipment inspection to identify fire suppression and detection systems are charged / operational
- Make sure a firewatcher is in place for hot work — and monitors for 30 minutes after the task
- Sign on before entry to Hazardous Areas
- Execute quality maintenance to prevent the risk of fire on equipment
- Ensure all chemicals used are approved for use onsite and controlled as per SDS
- Decant chemicals into suitable, clearly labelled containers
- Confirm all equipment is fit for purpose and tested & tagged

Lifting & CranageWhen conducting lifting tasks I will:
- Never enter a line of fire / drop zone - With consideration made to fall / equipment trajectory.
- Only use equipment that’s fit for purpose, inspected, tested and tagged
- Assess ground and environmental conditions for suitability and stability
- Establish work area and drop zone barricading to limit area only to workers involved in the lift

- Erect area and drop zone barricading to prevent unauthorised access to hazardous areas.
- Never enter a line of fire / drop zone - With consideration made to drop-zone / equipment trajectory.
- Only use fit-for-purpose equipment, that is inspected and suitable for use.

Vehicle IncidentWhen operating or working around vehicles and equipment I will:
- Maintain equipment separation distance requirements (50/30, 20/10, 10/3) at all times
- Confirm positive communication is achieved when interacting with other equipment
- Ensure the entry and exit conditions are achieved when inside an active excavator swing radius
- Always drive defensively, check blind spots and apply Hierarchy at intersections
- Wear a seat belt correctly while operating / traveling in vehicles and equipment
- Drive to conditions and follow speed limits
- Ensure my vehicle / equipment is safely parked
- Ensure that windrows are constructed and maintained to the correct standard
- Complete daily equipment and walk around inspections prior to use
- Ensure when spotting equipment into the workshop remain outside of the line of fire and always be visible to the operator
- Walk on designated walkways in carparks, buildings and on machines

Working in or around liquid bodiesWhen working on or around liquid bodies I will:
- Wear a life jacket at all times when on water
- Ensure that hard barriers are constructed and maintained to prevent inadvertent access
- Conduct Geotechnical Operational Checklist to identify hazards
- Check the depth of water prior to tramming a machine into water.
- All sumps must be barricaded and delineated prior to the excavator leaving the area
