Confined Space IncidentWhen required to work in a confined space I will:
- Only conduct confined space tasks if Trained, Competent and Authorised
- Identify, Isolate, Lock and Verify all energy sources prior to entry
- Develop a suitable Rescue Plan and ensure that all rescue equipment is in place
- Confirm my atmospheric gas monitor is bump tested and calibrated prior to use
- Conduct atmospheric monitoring before and during my task
- Ensure mechanical or natural ventilation is adequate to purge or ventilate any contaminant prior or during task

Dropped Object
When there is the potential for dropped objects during the task I will:
- Ensure the drop zone (consider deflection and 4:1 guide) is delineated by red and white danger tape
- Confirm all tools, materials and equipment are adequately contained to prevent objects from falling

Electrical Incident (High/Low Voltage)
When required to conduct electrical work I will:
- Identify, Isolate, Lock and prove De-energised, before conducting my task
- Not conduct electrical work unless Trained, Competent and Appointed

Engulfment Incident
When there is the potential for engulfment during the task I will:
(Stockpile operations)- Confirm stockpile condition and valve status
- Not travel over an operating coal valve
- Not operate with a Bench height exceeding cab height
- Conduct pre start inspections prior to operating mobile equipment on stockpile (Trenching)
- Install batter or shoring when trenching greater than 1.5mtrs
- Establish suitable barriers and barricading to clearly identify and control trenching work area (CHPP)
- Identify, Isolate (Dissipate or Eliminate), Lock and Verify all energy sources

Explosives Detonation
When working with or around explosives I will:
- Ensure no personnel are within the exclusion zone during blasting and lightning events
- Ensure blast demarcation and bunding is in place prior to the commencement of loading
- Ensure that a traffic management plan is in place prior to loading and adhered to throughout
- Only access, transport or handle explosives if Trained and Appointed
- Understand and follow the drill and loading plan where hot or reactive ground has been identified
- Immediately notify the shotfirer in charge of a suspected misfire
- Always have a shotfirer supervise any recovery or excavation involving misfires.
- Ensure separation of explosives while being transported and only assemble initiation explosives immediately prior to placing down the hole

Fall from HeightWhen working at heights I will:
- Protect the edge - Protect open edges through installation of hard barriers to prevent falls
- Only use fit for purpose equipment that has been inspected, tested and tagged
- Assess ground and environmental conditions for suitability and stability
- Develop effective rescue plans - Equipment is available and standby person trained

Fall of GroundWhen working around dumps, highwalls, low walls and stockpiles I will:
- Inspect my work area to identify geological hazards
- Follow primary stand off zones
- Install windrows for primary standoff zones. 10mtrs from the toe and 5mtrs from the crest at all times
- Ensure that windrows are constructed and maintained to the correct standard
- Dig and Dump to design

Fire & ExplosionWhen working on, in or around equipment and infrastructure with the potential for fire and/or explosion I will:
- Conduct work area inspection to identify ignition sources are adequately controlled
- Conduct equipment inspection to identify fire suppression and detection systems are operational
- Ensure a firewatcher is in place for Hot Work activities (monitor for 30 mins after task)
- Sign on before entry to Hazardous Areas
- Execute quality maintenance to prevent the risk of fire on equipment
- Ensure all chemicals used are approved for use onsite and controlled as per SDS

Hazardous Substances
When working with or around hazardous substances I will:- Ensure chemicals are approved in CHEMALERT and stored and used as per the SDS.
- Ensure when decanting chemicals, it is into a suitable container that is clearly labelled

Lifting & CranageWhen conducting lifting tasks I will:
- Never enter a line of fire / drop zone - With consideration made to fall / equipment trajectory.
- Only use fit for purpose equipment that has been inspected, tested and tagged
- Assess ground and environmental conditions for suitability and stability
- Erect area and drop zone barricading to limit area only to workers involved in the lift

Potable Water ContaminationWhen preserving the quality of drinking water for site I will:
- Monitor water quality test results and follow response protocols
- Inspect tanks, pipes, hoses and connections to ensure they are clean and protected from contamination

Remotely Piloted Aircraft IncidentWhen operating a remotely piloted aircraft I will:
- Be trained and competent in the operation of the make and model
- Review upcoming maintenance and complete pre flight planning
- Select a suitable take off and landing area and consider barricading
- Monitor the RPA during operation

Rotating & Moving Equipment IncidentWhen working around Tooling, Equipment and Components that are required to be energised when people are in the vicinity I will:
- Isolate - Prior to working on equipment.
- Follow the Exclusive Control process - When diagnosis and function testing is required
- Ensure guards / barriers / barricading is in place - To protect workers from line of fire & rotating / moving equipment.

Severe WeatherIn the event of severe weather I will:
- Adhere to the instructions of the supervisor in the event of a Red alert

Tyre & RimWhen working around tyres and rims I will:
- Identify and report suspected hazardous tyres - Remain 300m away from a hazardous tyre - staying clear of the extreme danger zone (between 2 and 10 o'clock).
- Will not enter barricaded areas - To protect personnel from line of fire/drop zones.
- Never enter a line of fire / drop zone - With consideration made to drop-zone / equipment trajectory.
- Only use fit for purpose equipment - Tyre/rims are designed for intended use, defect free, and used as per OEM guidance.

Uncontrolled Release of Stored EnergyWhen there is the potential for an Uncontrolled Release of Stored Energy during the task I will:
- Never enter a line of fire
- Identify, Isolate (Dissipate or Eliminate), Lock and Verify all energy sources
- Ensure barricading is in place to protect workers from line of fire

Vehicle IncidentWhen operating or working around vehicles and equipment I will:
- Maintain equipment separation distance requirements (50/30, 20/10, 10/3) at all times
- Confirm positive communication is achieved when interacting with other equipment
- Always drive defensively, check blind spots and apply Hierarchy at intersections
- Wear a seat belt correctly while operating / traveling in vehicles and equipment
- Drive to conditions and follow speed limits
- Ensure my vehicle / equipment is safely parked
- Ensure that windrows are constructed and maintained to the correct standard
- Complete daily equipment and walk around inspections prior to use
- Ensure when spotting equipment into the workshop remain outside of the line of fire and always be visible to the operator
- Walk on designated walkways in carparks, buildings and on machines

Working in or around liquid bodiesstrong>When working on or around liquid bodies I will:
- Wear a life jacket at all times when on water
- Ensure that hard barriers are constructed and maintained to prevent inadvertent access
- Conduct Geotechnical Operational Checklist to identify hazards
- Check the depth of water prior to tramming a machine into water.
- All sumps must be barricaded and delineated prior to the excavator leaving the area

Working alone in Remote and/or Isolated Areas
When working alone in remote and/or isolated areas I will:
- Ensure check-in and communication process is established with primary contact
- Submit working alone in remote and/or isolated areas checklist before commencement of task
- Carry a MAC issued Personal Emergency Device (PED)
